78 YEARS OLD · LONG-TERM FOLLOW-UP

CLINICAL OPENING

She first came when pain had taken over her life.

Mme. Marine first came in March 2025, at the age of 78. She was not simply experiencing discomfort. Her whole body appeared organized around pain, exhaustion, and protection.

A severe herpes zoster outbreak had spread across the left lumbar, abdominal, and pelvic region. The lesions were dense and visible. The pain extended into the left gluteal area, making walking difficult.

She arrived bent forward, tense throughout the body, unable to sleep properly, and discouraged after more than one month of persistent pain.

INITIAL SYSTEM STATE

The body was no longer moving freely. It was protecting.

Main Complaint

Severe left-sided post-herpetic pain involving the low back, abdomen, pelvis, and gluteal region.

Sleep State

Pain prevented restorative sleep. Medication brought only temporary relief.

Visible Posture

Flexed posture, guarded walking, strong posterior chain tension, and global muscular contraction.

First Shift

After the first session, she slept well that night and felt a meaningful reduction in pain.

PATTERN ATLAS

The clinical picture was more than local nerve pain.

Global Protective State

Pain, insomnia, flexed posture, and whole-body guarding suggested a system organized around protection.

Posterior Compression

Strong tension was observed through the back, sacral region, and cervical axis.

Autonomic Dysregulation

Sleep disruption, pain persistence, and emotional discouragement reflected nervous system overload.

System Exhaustion

After weeks of pain and poor sleep, the body had little reserve left for recovery.

BEFORE & AFTER CLINICAL ATLAS

What changed first was not only pain. It was the system.

Before

Bent posture, difficult walking, left gluteal pain, poor sleep, visible distress, and strong protective contraction.

After

Sleep returned, pain decreased, movement became easier, hope returned, and daily life gradually expanded again.

What changed first?
The body stopped organizing everything around pain.

ENTRY STRATEGY

The first work was to soften the protective chain.

1. Posterior fascial release

Work began from tense points in the back, where the body was strongly guarding.

2. Cervical connection

The posterior tension appeared to connect upward into the cervical region, suggesting a broader protective chain.

3. Local gentle release

The painful left-sided area was approached carefully, without forcing the system.

CLINICAL TURNING POINT

That night, she slept.

After the first session, Marine reported that she slept well that same night. Pain was also noticeably reduced.

This first change was important because it gave her something she had lost: the feeling that recovery was still possible.

Over the following sessions, her pain continued to improve. After more than ten sessions, the post-herpetic pain had disappeared.

WHAT BECAME VISIBLE

Life became larger again.

Sleep

She began sleeping again after the first session.

Walking

Movement became easier as pain and guarding decreased.

Daily Life

She returned to shopping, cooking, gardening, and household tasks.

Family Role

She continued caring for her husband and later helped with her granddaughter.

Travel

She travelled three hours by train to Paris and stayed one week.

Trust

She and her husband developed a deep long-term therapeutic trust.

CLINICAL REFLECTION

For an elderly person, healing is measured by life.

In this case, the most meaningful result was not only the disappearance of post-herpetic pain.

The deeper clinical meaning was the restoration of function: sleep, walking, confidence, autonomy, family participation, and the ability to continue living actively.

“She did not simply have less pain. She returned to the rhythm of her own life.”

KEY LEARNING POINTS

What this case teaches.

Post-herpetic pain may involve a whole protective system.

The visible rash was local, but the response involved posture, sleep, movement, and emotional state.

Sleep can be an early sign of system regulation.

Her first meaningful change was a good night of sleep, before the full pain pattern had resolved.

Function matters more than numbers.

Shopping, gardening, travelling, and caring for family became more meaningful than a simple pain score.

Long-term care reveals the real story.

With nearly 47 appointments, this case shows not only recovery, but maintenance, trust, and aging with vitality.
